Adult Day Health Centers: Adult day health centers offer programming for people with care needs during the day. Services are tailored to the individual and may include therapeutic activities, transportation, meals, and personal care. The programs can help give a break to a round-the-clock caregiver.

Caregiver: A person caring (physically and/or emotionally) for another person or loved one, also called a caretaker or care partner.

Community Tables: Formerly known as “congregate meal sites”, Community Tables offer an opportunity to meet new or old friends and engage in social activities while having a nutritious meal. Community Tables locations include senior centers, churches, and housing communities.

Dementia: A general term for a decline in mental ability severe enough to interfere with daily life. Memory loss is an example. Alzheimer's is the most common type of dementia, but not all dementia comes from Alzheimer's disease.

Discharge planner: A professional who assists patients and their families in developing a method of care for a patient following a hospital or nursing home stay.

Medicare: A federal health insurance program for adults with disabilities and all older adults 65+

Medicaid: A state and federal health coverage program for individuals with low income

Respite: A short period of rest or relief
